Documentary Requirements
Applicants are required to submit the following:
- 1×1 ID photos – 3 pieces
- Original Certificate of Good Moral Character
- For Senior High School graduates: Original SF10 (School Report Card)
- For college undergraduates: Original Transcript of Records (TOR)
- Original Certificate of Low Income
- Photocopy of Birth Certificate
- Original Medical Certificate indicating FIT FOR TRAINING
- Long brown envelope – 2 pieces
